If you are interested in hosting a CHD Awareness Quilt, we ask that you read the Hosting,Shipping, and Handling Policies for more information on hosting a quilt and requirements and submit the following form. By filling out this form, you are showing interest in hosting one or more the CHD Awareness Quilts. The information from the form will be shared only with the CHD Quilt Council. The CHD Quilt Council will review all forms. Hosting is determined on a first request basis, type of function quilt will be displayed, and the dates available. If chosen to host a quilt, you will be contacted with further information. Please feel free to email us with questions.

Filling out this form does not guarantee that you will host a quilt.

If chosen to host a quilt, we cannot guarantee that the quilt you receive will have yours/your child's name on the quilt. We will do our best to try to make this possible.

We ask that hosts pay a $ 50.00 Hosting Fee for each quilt requested and pay for shipping of the quilt back to the CHD Awarness Quilt Project, Inc. We ask that the Hosting Fee must be paid 30 days in advance of your event. A Hosting Fee form will be sent to you upon approval of your request. A quilt will not be shipped until we receive the payment.

The Congenital Heart Defect (CHD) Awareness Quilt Project, Inc. is a non-profit (501c3) organization dedicated to promoting greater public awareness of congenital heart defects while honoring and remembering the more than one million people born with a chd.
